NASHVILLE, United States – Tennessee failed to execute death-row inmate Christa Gail Pike on Wednesday after two doses of pentobarbital failed to result in her death, prompting Gov. Bill Lee to halt the state’s remaining executions for 2026 and order an independent investigation.

Pike, 50, remained alive after the second dose and was taken by ambulance from Riverbend Maximum Security Institution to a hospital. Her attorneys said they were not initially informed about her medical condition.

The failed execution came hours after the U.S. Supreme Court overturned a last-minute stay issued by a federal appeals court, allowing Tennessee to proceed.

Two doses fail during execution

Media witnesses reported that the execution chamber opened at about 7:27 p.m. local time, with Pike strapped to a gurney.

According to the Associated Press, Pike remained awake during the procedure and at one point raised her head and asked whether the sensation in her arm was supposed to feel the way it did.

A second dose of pentobarbital was administered by about 8:26 p.m.

Witnesses later heard Pike snoring behind the closed curtain. At approximately 8:53 p.m., the microphone was cut off and media witnesses were instructed to leave.

Pike’s lawyers subsequently filed emergency motions seeking to stop the execution and obtain immediate medical treatment, arguing that she remained alive after the injections.

The Tennessee Department of Correction said officials followed every step of the state’s established execution protocol, which had been approved by the state attorney general’s office.

State protocol provided for a second dose

Tennessee’s lethal-injection protocol calls for a second set of drug syringes if an inmate has not been determined to be deceased after the first set.

The protocol does not specify what officials should do if an inmate remains alive after the second set of injections.

Pike’s attorneys have separately challenged Tennessee’s lethal-injection procedures in court. Their filings have raised concerns about intravenous access, the execution drugs and potential medical risks associated with the protocol.

Those concerns are allegations made by the defense and have not been established as findings by a court.

The state has disputed any characterization that its officials failed to follow the approved procedure.

Governor halts remaining executions

Following the failed execution, Lee announced that Tennessee’s remaining scheduled execution for 2026 would not go forward.

He ordered a comprehensive third-party review to determine what happened during the attempt. Lee said carrying out a lawful death sentence was among the state’s serious responsibilities and that executions should be conducted legally, constitutionally and effectively.

The decision follows another failed execution attempt in Tennessee earlier this year.

In May, officials called off the execution of Tony Von Carruthers after they were unable to establish the required intravenous access. Lee later granted Carruthers a temporary one-year reprieve.

The two cases involved different circumstances, but both resulted in Tennessee being unable to complete a scheduled lethal-injection execution.

Supreme Court had allowed execution to proceed

Pike’s execution had already been delayed earlier Wednesday.

The Sixth U.S. Circuit Court of Appeals issued a temporary stay shortly before the scheduled 10 a.m. execution while considering claims involving evidence of sexual abuse and neglect that Pike’s lawyers argued had not been adequately considered during her original sentencing.

Tennessee appealed to the U.S. Supreme Court.

The Supreme Court subsequently vacated the Sixth Circuit’s stay, allowing Tennessee to proceed with the execution. Justices Sonia Sotomayor, Elena Kagan and Ketanji Brown Jackson dissented.

The Supreme Court’s action concerned the stay and did not resolve all of Pike’s underlying legal claims.

Pike remains on death row

Pike was convicted of the 1995 murder of 19-year-old Colleen Slemmer in Knoxville. Pike was 18 at the time of the killing.

The Tennessee Supreme Court set Sept. 30, 2026, as Pike’s execution date after determining that there was no legal reason preventing enforcement of the death sentence and denying her request for a certificate of commutation.

Tennessee Department of Correction records continue to list Pike as a death-row offender.

Pike had been scheduled to become the first woman executed by Tennessee in more than two centuries. That did not happen because the lethal-injection procedure was not completed.

Pike’s medical condition remains unclear

After the failed execution, Pike was transported to a medical facility.

Her lawyers said they were not initially informed about her condition. Public reporting has not established a definitive medical assessment of the effects of the two injections.

Witnesses and Pike’s attorneys have described her as remaining conscious or producing sounds after the second injection, while the state has maintained that its personnel followed the established protocol.

The independent review ordered by Lee is expected to examine what happened during the execution attempt and whether state procedures were followed as required.

Legal questions remain over any future execution

The failed execution has created a new legal and procedural situation for Tennessee.

Pike’s attorneys have already sought emergency relief, arguing that the failed attempt raises constitutional questions about any future effort to execute her. Courts will determine how those claims affect the state’s authority to carry out the existing death sentence.

The state has ordered an external review rather than immediately scheduling another execution.

For now, the established outcome is that Tennessee attempted to execute Pike by lethal injection, administered two doses of pentobarbital, did not establish her death, transported her to a hospital and halted further executions scheduled for the rest of 2026.

The execution remains incomplete, and Pike remains under a death sentence.
